FELLOW IN LAPAROSCOPIC COLORECTAL SURGERY

The Post

This post is funded primarily by Ethicon Endosurgery as part of the Department of Health initiative to establish National Training centres for laparoscopic colorectal surgery.

Laparoscopic colorectal Fellow

¨ Further their experience in laparoscopic colorectal surgery. It is anticipated that at the end of the 6 month period the appointee will have performed or assisted with approximately 50 major laparoscopic procedures. It is expected that at the end they will be capable of independent Consultant practice in this area.

¨ Assist in the inception of a Department of Health funded laparoscopic colorectal cancer course. The Fellow will be specifically responsible for:

a. Assisting in course design and delivery.

b. The setting up of original research in this area, and establishing ethics

c. Liaising with theatres and the simulation centre to facilitate the smooth running of courses.

d. Attendance at other Colorectal Units nationally as required. It is likely that some laparoscopic colorectal operating will be performed in centres away from QMC as the course evolves.

e. Attendance at national steering group meetings as required.

The post would be ideal for a post CCT candidate seeking to establish a specialist career in Laparoscopic colorectal surgery with an interest in medical education, whilst awaiting an appropriate Consultant post to become available. THIS POST IS NOT RECOGNISED FOR TRAINING.

The Colorectal Unit at NUH

Colorectal Services for Nottingham are all located on the Queens Medical Centre campus of Nottingham University Hospitals. The unit consists of 5 NHS Consultants (Abercrombie, Armitage, Maxwell-Armstrong, Robinson and Williams), and two from the University of Nottingham (Professor JH Scholefield and Mr A Acheson). The unit also includes 7 Nurse Practitioners and 5 Specialist Registrars. 550 colorectal cancers are managed per year, making NUH the third busiest in UK. Approximately 25% of all cancers are resected laparoscopically, with approximately 80 - 100 major procedures performed per year. The current programme has been in place since July 2003. In addition there is an active TEMS and colonic stenting service. Referrals from Mid-Trent are taken for anal cancers, intra-abdominal sarcomas, complex enterocutaneous fistulae, faecal incontinence requiring sacral nerve stimulation and complex fistula in ano. The colorectal unit participates in the General Surgical on call rota, as well as providing a weekday 9-5 rota for colorectal surgical emergencies at the City Hospital campus of NUH.

Nottingham

Nottingham is a thriving city in the East Midlands with a cosmopolitan culture and thriving nightlife. Good rail services exist to London (1hr50 minutes) and Europe through the Eurotunnel hub at St Pancras station. In addition East Midlands airport 10 miles from the city centre provides flights to a range of European destinations. There are good private and state schools in the area and the city is within easy reach of areas of outstanding natural beauty such as the Peak District. The City has a Test Match cricket ground (Trent Bridge), a Division 1 rugby club and 2 football teams.

Lines of Management

The post-holder’s immediate line managers will be Mr Austin Acheson (Associate Professor), Mr Charles Maxwell-Armstrong (Colorectal Service Lead and Lead for Undergraduate General Surgical Education) and Dr Bryn Baxendale (Consultant Anaesthetist and Director Mid-Trent Simulation Centre). The appointee will ultimately be responsible to the Clinical Director of the Digestive Diseases and Thoracics Directorate (Mr N Welch).

MAIN CONDITIONS OF SERVICE

The post holder is expected to be aware of local policies and procedures and take note of the standing orders and financial instructions of the Nottingham University Hospitals NHS Trust. The post is covered by the Medical and Dental Terms and Conditions of Service.

Salary

The salary will be payable on the current nationally agreed Str scale. There may be opportunities for on call/locum payments.

This post is funded by Ethicon Endosurgery as part of the National Laparoscopic colorectal cancer training programme initiative.

Annual Leave

The Laparoscopic/Teaching Fellow shall be entitled to annual leave at the rate of 27 days per annum.

Tenure

The post is offered for 6 months in the first instance with a possible extension for a further 6 months.

Qualification & Experience

The post holder must have full registration with the GMC. It is no longer a requirement of employment within the NHS that medical staff be registered with a Medical Insurance organisation. Should you wish to maintain registration with such as organisation, this is entirely your own choice. Because the post is not recognized for training it is likely that the appointee will be post CCT (Completion of Certificate for Training)

Because of the nature of the work for which you are applying, this post is exempt from the provisions of section 4 (2) of the Rehabilitation of Offenders Act 1974 by the virtue of the Rehabilitation of Offenders Act 1974 (exemptions) Order 1975. Applicants are, therefore, not entitled to withhold information about convictions which for other purposes are ‘spent’ under the provisions of the Act, and in the event of employment failure to disclose such convictions could result in dismissal or disciplinary action by the Trust. Library Facilities

QMC contains a comprehensive medical library. The library has links with the University Library in Nottingham. The library carries up-to-date Medline and Cochrane databases.

Accommodation

There is accommodation at QMC but it is envisaged that the post-holder will make his/her own arrangements.

Nottingham NHS Treatment Centre

In 2008 the new Nottingham NHS Treatment Centre will be opening on the QMC campus. The Treatment Centre will be managed by Nations Healthcare - an Independent Sector Provider whose parent company is Circle. When the Treatment Centre opens much of the activity undertaken within it will be transferred from Nottingham University Hospitals NHS Trust.

In terms of the effect of this on NUH staff, the majority of staff and the work they undertake will not be affected at all. However other staff may be affected in one of two ways:

  1. You may be required to undertake most of your work in the Treatment Centre.
  2. You may be required to undertake some of your existing work in the Treatment Centre.

Nursing and Medical staff required to work within the Treatment Centre will be seconded to do so. Other groups of staff will work there under the terms of a Service Level Agreement between the Trust and Nations Healthcare. (eg physiotherapy staff, admin staff, hotel services staff etc).

All staff that are currently NUH employees and are required to work in the Treatment Centre will remain the employees of Nottingham University Hospitals NHS Trust and their terms and conditions of employment will remain unaffected.
